Residential Care and Post-Discharge Support

The UYH facility is a residential home with 5 beds and a 1:5 staff-to-child ratio, with the potential for a 6th child. Nurses are available 24 hours a day and 7 days a week, and the agency's clinical services are tailored to meet the individual needs of each child. UYH also provides discharge planning and support for post-discharge aftercare services, if the child's family teams determine it is necessary. The agency will employ strategies and approaches to ensure safe discharge planning and aftercare, including developing a discharge plan that is in accordance with FFPSA and offering at least six months of support after a child is discharged.
